It includes information such as the reason for taking each medication, the dosage, frequency, and any potential side effects. The plan is created by a healthcare professional and is intended as a reference for all members of the patient's care team.
The main goal of a Medication Management Plan is to ensure the patient is taking the correct medications at the right time, and in the right doses, to achieve the desired therapeutic outcomes and prevent adverse events. The plan can be updated regularly as needed, and is an important tool for medication management.
The process includes:
- Evaluation of current medication usage
- Comparison and update of the medication list
- Examination of the patient's medication regimen by a healthcare professional
- Monitoring drug effectiveness and potential adverse reactions
- Management of any negative effects caused by medication
